I am an in-network service provider for the following insurance plans:

• Aetna
• Beech Street

If I am in your network, you are responsible for your copayment and I will bill your insurance company for the balance.  If I am not in your plan's network, you are responsible for payment at the time of service, and I will give you a statement with all the information that you need to obtain out-of-network reimbursement.  Typically, it ranges from 50% to 80% after a deductible is met.

Insurance benefits (such as deductibles, copayments, and number of sessions) vary according to plan and employer.  Your employer may also revise your benefits during the course of treatment.  Please note that you are responsible for finding out your plan benefits and fees and keeping track of any changes.

If you have any questions about your insurance, insurance coverage and fees, please contact me and we can discuss it prior to your first appointment.

I will not release any information about you without your written permission.  At times, it is helpful for me to speak to another professional, such as your primary care doctor or a psychiatrist, but to do that I will need to discuss it with you first and get your permission. In addition, in order to obtain reimbursement from your insurance company, it is necessary to provide diagnostic information and sometimes treatment information.

There are, however, rare exceptions to confidentiality, as mandated by law. These exceptions include situations when someone is a threat to themselves or others, or when there is an indication of ongoing child abuse.

Your privacy is essential to your comfort and trust in psychotherapy, and I will do everything I can to safeguard it.
